This report is a regional summary of consistent discharge code data, collected by 20 Adult Day Program provider agencies across Champlain over a 6-month period. The data was collected from August 2018 to January 2019, and compiled by CCSN in early March 2019.

A total of 503 individuals were reported as discharged from ADP programs across

the region during this initial 6-month period.
